Job Description Summary
The Steven A. Cohen Military Family Clinic (Cohen Clinic) at the University of Pennsylvania is seeking a full time Care Coordinator. The Cohen Clinic provides no cost, evidence-based behavioral health care to US military veterans, service members, and military family members, including individual therapy for children, adolescents, and adults, as well as couple and family therapy, in an interdisciplinary setting with Case Management and Medication Management.

Job Description

The Care Coordinator will serve as an active member of our care team, directly supporting clients in establishing and navigating care, maintaining a timely and accurate health record, and taking the lead on data entry, cleaning, and management for program evaluation initiatives. Under the supervision of the Assistant Director and in collaboration with the Intake Coordinator and Case Manager, the Care Coordinator will complete structured clinical and diagnostic phone interviews, coordinate initial evaluations, and maintain timely and appropriate documentation to support our growing client base. They will participate in weekly interdisciplinary clinical team meetings, team meetings, and engage in training on evidence-based practices, military cultural competence, and more. The Care Coordinator will assume primary responsibility for maintaining accurate entry of intake data for clinical use and quality initiatives. Finally, the Care Coordinator will serve as second backup for our Client Services Associate, providing the highest quality of care to clients, new referrals, and other stakeholders by greeting and orienting clinic visitors and callers, facilitating check ins, etc.

The population treated at the Cohen Clinic is multicultural and diverse, including with respect to race, ethnicity, gender identity, sexual orientation, ability, and socioeconomic status. Team members serve underserved groups of veterans and service members, including women veterans and LGBTQ+ and family members. Accordingly, applicants with experience and/or interest in multicultural/diversity issues in and working with diverse populations would be a good fit for the position. Interested staff also have the opportunity to engage in a Racial Equity Working Group that meets monthly, ongoing consultation with external facilitators, and staff-driven initiatives to continuously improve racial equity, accessibility, and safety for our team and all we serve.

The Cohen Clinic is currently operating on a hybrid schedule with 1-2 days of remote work available to all team members. Position requires one evening per week until 7pm and some additional non-standard hours per client needs (e.g., occasional mornings at 8am).

Qualifications
  • Minimum of a high school diploma and 3-5 years of experience or an equivalent combination of education and experience required.
  • Bachelors degree in psychology or a related field preferred.
  • Experience in a healthcare setting or research lab preferred.
  • Must pass Pennsylvania Child Protective Services Act (also known as Act 73) background checks, within 30-90 days of employment.
  • Experience in the military, experience working with veterans or military service members, or demonstrable interest in serving this community is strongly preferred.
